I have seen Windows Update fail to update on lots of Windows 2003 installations.
Especially if the server are being deployed using Provisioning Services.
You check the 3 main services are started:
- Automatic Updates
- BITS
- EventLog
But regardless of you actions updates fail.

In many cases the re-regsitering of the Windows Update dll's is the solution but there are other possibilities that are worth noting.
